The IMATIS patient portal is a new take on the IMATIS Fundamentum web portal, designed for patients to gain access to information, communicate with staff and participate actively in their own treatment. Any healthcare provider, regardless of level and frequency of care provided, can benefit from offering the Ludus patient portal to their patients or clients.

IMATIS Ludus Patient Portal

Staff members must have access to the same portal in order to manage the data coming in from and going out to the patients. Electronic whiteboards are used to organize patient groups, or - in hospitals - wards and departments. When used across different units and health organizations within a region or national setting, the whiteboards can track and manage a patient's entire journey. The IMATIS Portal is used by to manage the patient portal, update facility information, allocate staff to patients, enter new information, allocate staff members to patients etc. Patients can be contacted directly from the whiteboard, including sending appointment reminders via text messages.

The system can show a comprehensive view of practical information, individual health information and progress reports as well as tools for patients who require a degree of monitoring in their own homes.

The IMATIS system makes extensive use of integrations with primary systems, in order to present the user with a tailored set of data. The IMATIS Portal is used both as a staff portal and patient portal, with different user interfaces and application/information access levels. The staff-side system consists of a web portal with number of organizations/units with a number of whiteboards.

The IMATIS Ludus patient portal can assist the patient in:

     ·     Showing appointments and referrals and receive reminders of appointments

     ·     Showing allocated personnel and care team, including important contact numbers and addresses

     ·     Browsing personal health information, conditions, allergies, clinical instructions, family history etc.

     ·     Browsing general information material, such as disease information and coping techniques

     ·     Viewing own clinical pathway progress (standardized clinical pathways or personalized pathways)

     ·     Responding to questionnaires and other surveys or fill in information required for forms

     ·     Operating medical devices or sensors for recording of device data, to be transferred to the GP or hospital

     ·     Managing pharmacy contact and prescriptions.

Patient Surveys and Questionnaires

Questionnaires such as daily health reports are created in IMATIS Forms module, which was designed to be used for remote monitoring of COPD patients recently released from hospital. The patients use tablets, smartphones or computers to answer the surveys.

A survey is built in the Forms builder configuration, which is a low-level configuration module where anyone with the correct access levels can build a survey in a manner of minutes, using dropdown menus and entering questions and alternative paths for certain questions, if required. Surveys can be used to score the answers and pre-process them automatically. Adding process filters for survey responses with different actions to complete for the groups of patients, depending on criticality etc., to be followed up according to established routines.
